Terms & Conditions*
- In the event that I am no longer able to keep this pet, I will not give them away, sell or leave the pet behind. I will contact BAS to discuss further actions. I understand BAS will accept will accept the animal back.
- Ownership Agreement- As the new owner, you agree to take full responsibility for the care and wellbeing of this pet for its lifetime. Once adoption is finalized you will be responsible for all expenses and medical bills regarding this animal.
- BAS will not be responsible for the dog's vaccinations or medical needs after this point.
- Medical Voucher- A voucher may be given to help offset the cost of getting the animal fixed. Please read the given paperwork thoroughly.
- It is the owners responsibility to keep track of this dog's health records. BAS is not responsible for lost or damaged paper work.
- I have read and understand this adoption contract ad certify that the information provided is true and correct. I will take full responsibility for this dog's health and will discuss this with a veterinarian of my choice. I also release Beaverhead Animal Shelter from all responsibilities from this time forward.
